The insured first pays most of medical expenses up to the Annual Deductible amount. Then he/she shares a portion of the medical costs with the plan until his/her total payments reach the Annual Out of Pocket (OOP). Then the insurance pays 100% of the medical cost for the rest of the year.

Your deductible payments are accumulated in one year period (either policy year or calender year). It starts over for the new period.
